Primary Care Network
About
In collaboration with Noojmawing Sookatagaing Ontario Health Team, the Primary Care Network (PCN) is dedicated to improving health outcomes by connecting primary care clinicians, agencies, and community partners. We embrace co-design principles to ensure that our initiatives are driven by locally-identified needs and reflect the diverse perspectives of providers, patients, and communities. The PCN advocates for equitable access to healthcare, sustainable practices, and the advancement of population health, with a focus on addressing health inequities and ensuring the ongoing wellbeing of both providers and patients.

Core Functions
The PCN will focus on advancing locally-identified health priorities, improving the coordination of care, and advocating for resources and policies that support primary care practitioners and the patients they serve. The core functions include:

Embed Primary care voice and clinical leadership within Noojmawing Sookatagaing OHT.

Facilitate the creation of care pathways that connect primary care, hospitals, specialists, and community-based services. This co-design process ensures that care pathways are relevant, accessible, and reflect the needs of the local population.

Actively advocate for funding, programs, and policy reforms that prioritize the needs of primary care providers and patients. This includes addressing funding models, health system navigation, and the integration of services to improve healthcare delivery.

Implement initiatives to reduce health inequities by targeting underserved populations, improving access to care, and supporting healthcare services that are culturally and linguistically appropriate.

Develop and promote strategies to manage population health, including data collection, community health assessments, and evidence-based interventions that address the social determinants of health.

Provide primary care practices with the tools, resources, and training needed to operate sustainably, including digital health tools, funding advocacy, and organizational support to improve efficiency and reduce burnout.

Facilitate better navigation across the health system by improving access to integrated services and helping both patients and providers understand and utilize available resources.

Support the ongoing education of primary care practitioners, offering training on leadership, emerging health issues, digital tools, and the latest evidence-based practices.
